The etiology of Henoch-Schonlein Purpura (HSP), a small-vessel systemic vasculitis in children, remains elusive; its pathogenesis is listed as an immune-mediated vasculitis, associated with IgA deposition. We have examined a 4 years old girl presented with HSP 8 days after her first flu vaccine. Hypothesis We hypothesized that HSP might be an adverse reaction to influenza vaccination. Results A healthy 4-year-old girl, reported to the ER four days after the appearance of rash and bruising. She was diagnosed with HSP based on clinical symptoms and microhematuria, 8 days after she had received her first-ever seasonal flu vaccine. A literature search revealed 19 reported cases of HSP soon after vaccination with influenza vaccine. Scrutiny of the Ministry of Health registries in Israel revealed no similar reports related to the same batch of vaccine or any other batches, since the introduction of influenza vaccines in 2014. Conclusions HSP might be an adverse event post influenza vaccination. Caution should be used when administering future influenza vaccines to those who have had HSP post influenza vaccination. Patients should be informed that they may experience similar adverse reactions upon re- administration of the vaccine, and use vaccines formulated by a different manufacturer.
